• Teaching Certificates/Credentials (where applicable)
• College Transcript
• I-9 Form
• W-9 Form
• FBI Fingerprint Background check performed at DCPS
• SafetyNET Fingerprint Background check performed in-house
• Signed Confidentiality Statement
• Signed Media & Liability Release Statement
NOTE: You will also need proof of a negative tuberculin test or chest x-ray (from Doctor’s office)
